Tanaiste and Minister for Health and Children Ms Harney ; : I propose to take Questions Nos. 152 and 221 together. The Madden Report on Post Mortem Practice and Procedure, which I published recently, contains some 50 recommendations. Some of the recommendations are exclusively the remit of my Department, for example, the formulation of human tissue legislation, others are a matter for the HSE, for example, an audit of organs retained in hospitals, and others fall under the aegis of other agencies, for example, the coroners legislation is a matter for the Department of Justice Equality and Law Reform. My Department is examining each of the recommendation in consultation with relevant bodies. I have asked for an action plan to be prepared for the implementation of the recommendations and progress made will be closely monitored. EU Directive 2004 23 EC which sets quality and safety standards for procurement, testing, processing, preservation, storage and distribution of tissues and cells is in the process of being transposed into Irish law. This legislation will only deal with tissues and cells for human application. As regards stillbirths and maternity hospitals, Dr. Madden had access to all of the information collected by the Dunne inquiry, with particular reference to the issue of pituitary glands which is dealt with in chapter six of her report. As recommended in the report, I establishing a working group to examine issues pertaining to non-paediatric post mortems, that is, babies who died before or at birth, minors and adults. I have asked Dr. Madden to chair the group and will announce the full membership shortly. Hospital Staff. 153. Community-based epidemiological studies have provided important information on the demography of CHF, providing insight into the impact of CHF on public health13. CHF is often accompanied by a range of concomitant disorders that both contribute to the cause of the disease and play a key role in its progression and response to treatment. Information from large-scale multicentre trials on the most common co-morbidities, ischaemic heart disease IHD ; , hypertension and diabetes mellitus, provide guidance in therapeutic decision-making process. Furthermore, as CHF is often an endpoint in intervention trials of both hypertension and diabetes these studies afford important information on the prevention of CHF in these common diseases14. There are sex-based differences in co-morbidities in patients with CHF with females tending to be older, to have associated diabetes mellitus and hypertension, and to have more preserved ventricular function14. Sex-based differences in mortality have generally not been observed in females treated with angiotensin converting enzyme ACE ; inhibitors and beta blockers. In the digitalis intervention group DIG ; trial females did have a higher morbidity than their male counterparts15. This observation may have resulted from the higher plasma concentration of digoxin found in female patients. Age does not appear to influence the beneficial effects of ACE inhibitors or beta blockers in chronic systolic heart failure although tolerability of treatment may decrease with advance in age. Ongoing trials are currently addressing therapeutic interventions in elderly patients more representative of those treated in the community with CHF16, 17.

The number of patients withdrawing completely from study medication before the end of the study, for any reason other than death, was 71 33% ; in the suloctidil group and 67 31% ; in the placebo group. The primary reasons for such withdrawals are summarised in table 4. It can be seen that the numbers withdrawing for the various reasons were similar in the two groups, except with respect to side effects. Side Effects of the Study Treatments The number of patients reporting any side-effects at any time during the course of the study was 67 31% ; in the suloctidil group and 29 13% ; in the placebo group; this difference is statistically highly significant p 0.001 ; . Alleged side-effects were sufficiently severe, troublesome or numerous to cause complete withdrawal from study treatment in 29 patients in the suloctidil group and 12 patients in the placebo group; again this difference is statistically significant p 0.01 ; . The principal reasons for such withdrawals are summarized in table 5 from which it can be seen that the most striking differences against the active treatment are for vertigo dizziness 5 v 0 ; , constipation 4 v 0 ; and suspected hepatitis 6 v 1 ; The latter adverse reaction is potentially the most serious. A careful review of the charts of every patient in the study was subsequently carried out which disclosed two additional cases, both on placebo, for whom the word jaundice or hepatitis appeared in the patient record. The complete charts for each of these nine cases of suspected drug-related hepatitis were reviewed carefully, and without knowledge of study treatment, by a gastroenterologist S.M.C. ; . The conclusions reached by this independent reviewer were that study druginduced hepatitis was unlikely in one case on placebo, and most unlikely in the other two cases on placebo. For the six cases on suloctidil, the corresponding conclusions were that it was unlikely to be study drug related in one case, extremely unlikely in another case, suggestive in one case and highly suggestive in the and artane.

Many adverse events that occur in the setting of combination antiretroviral therapy are newly recognised and their pathogenesis is incompletely understood. Substantial uncertainty remains in relation to the relative contribution of HIV infection and specific antiretroviral agents, classes and combinations to the development of toxicity. Na reabsorption in the TAL but may also affect the Cl~ -sensing mechanism between the macula densa and the afferent arteriole, in order to avoid a change in filtration rate. In the mouse, the BSC2 Na-K-2C1 cotransporter is expressed in the terminal portion of the mouse inner medulla, on the basolateral membrane of the collecting duct epithelial cells. In this segment, the cotransporter may participate in several important functions: 1 ; Na + and or K + secretion, 2 ; acid secretion, and 3 ; cell volume regulation. Although the IMCD segment is generally believed to mediate Na + reabsorption through apical Na + channels and basolateral Na + K pumps [26], it has also been reported that atrial natriuretic peptide ANP ; stimulates NaCl secretion via a frusemide-inhibitable Na-K-2C1 cotransporter [27]. These results are consistent with the upregulation of the mIMCD-3 cell Na-K-2C1 cotransporter expression by ANP [28], a hormone otherwise known to downregulate the expression of the Na + channel and the Na + K pump [29, 30]. Thus, IMCD cells, under the proper hormonal signals, may switch from a Na + absorption mode to a Na secretory mode by regulating the expression and or activity of apical and basolateral transport mechanisms see Figure 4 ; . The Na-K-2C1 cotransporter on the basolateral IMCD cell surface could also play a role in K + secretion which has been demonstrated in physiological transport studies. The task of deciphering membrane characteristics from input measurements on a cylindrical cell is especially difficult when the membrane is nonlinear. This problem arises from variations in membrane state along the length of the cylinder and would vanish if it were made sufficiently short. Although evident in principle from cable theory, this dependence of input characteristics on length has never been clearly demonstrated by calculation and experiment. In the work described, the different input characteristics for long and short synthetic strands of cardiac muscle were closely matched by the different input characteristics of long and short theoretical cables with identical membrane properties. These input characteristics included not only the steady-state current voltage relationships, but also the time course of displacement in membrane potential after momentary voltage clamp at a point during repolarization of an action potential. Long cables showed qualitative features not present in the membrane description, such as an unexpected inward current. The input characteristics approached the membrane characteristics for fiber lengths less than 1 5 of the resting DC length constant.
