Lack of training, unfamiliarity with treatment options, out-of-stock drugs, broken equipment, sporadic electricity and water, and transportation challenges all threaten the health of women grappling with unsafe abortion23. Perhaps the greatest danger of all is indifference--or overt disdain. The lack of commitment on the part of medical and nursing staff to provide prompt, attentive and emotionally supportive care indirectly dooms women whose lives could easily be saved. Many women who reach medical facilities are met with suspicion and hostility, and their treatment deferred while other more `suitable' candidates receive medical attention10. Reproductive Medicine Service, Institut Universitari Dexeus, Barcelona, Spain Introduction: Checking for pronuclear formation in oocytes is usually performed 18-20 h after ICSI. One, two or three pronuclei can be observed at that time. Unipronucleate oocytes are considered to be unfertilized parthenogenetically activated ; , bipronucleate oocytes are considered to be normally fertilized, and tripronucleate oocytes are considered to be abnormally fertilized non-extrusion of the second polar body ; . The aim of this study was to evaluate the time-course of pronuclei formation and pronuclear development in human oocytes after ICSI. Materials and methods: Patients were treated with ICSI for severe male factor infertility, or fertilization failure or a low fertilization rate in previous IVF cycles. Details from a total of seven trials were therefore included in the systematic review, including a total of 846 people randomised to receive cinacalcet. The trials were largely well designed. The primary outcome for all the trials was a measure of serum PTH reduction. Only one paper provided information about patient-based clinical outcomes. This used retrospective analysis of adverse effect data from the four main RCTs to assess the impact of cinacalcet on fracture, cardiovascular events, parathyroidectomy and mortality. However, most of these data were based on 6-month follow-up and it is unclear how the results should be extrapolated to the longer term. Some data came from people who agreed to take part in an extension study after the original 6-month deadline and it is not known whether their characteristics were the same as the originally randomised population. Methods used for censoring in the analysis were unclear. In addition, death rates in the trials were half that reported for a similar age group by the UK Renal Registry. It is therefore unclear whether the results are applicable to the routine clinical population and clove.
