It is important that you have a safe and functional discharge plan before you leave the hospital. Other discharge options include inpatient rehabilitation and sub-acute extended care facilities. Some patients need more rehabilitation before they are safe for discharge home. If you and your orthopedic team decide you need additional rehabilitation, your discharge planner will work on transferring you to one of these specialized rehabilitation units.
