Edema of the cardiac muscle has attracted very little attention. In beriberi there is real edemia of the heart muscle. The electrocardiogram shows no anomalies, parallel to the greater content of tissue fluid, only the P-Q distance is regularly shortened. There are some other rare diseases with edema of the cardiac muscle. The heart is the only organ that by its function regulates its own tissue fluid movements. The problems of tissue fluid movement are discussed.

Albino rats, aged three months, were equally divided into four groups : Control C ; , the group exposed to restraint stress R ; , the group treated with sulfite S ; and the group exposed to stress and treated with sulfite RS ; . Rats were exposed to 1 hour of restraint stress daily for 21 days by placing the animals in a 25x7 cm plastic bottle .Sodium metabisulfite 520 mg kg day ; was given by gavage to the S and RS groups for 21 days. After the end of the experimental period, Cu, Zn-superoxide dismutase Cu, Zn-SOD ; , glutathione peroxidase GSH-Px ; , catalase CAT ; and thiobarbituric acid-reactive substances TBARS ; levels of brain were measured. TBARS levels were significantly increased in all experimental groups with respect to the C group, but higher in the RS group than in the R and the S groups. Cu, Zn-SOD activity was found to be decreased in the R group, but increased in the S group pared with the C group. However, in the RS group it was unaltered. Brain GSH-Px activity was significantly decreased in the R and S groups with respect to the C group. Statistically significant decrement in the brain Cu, Zn-SOD level was not detected in the RS group compared with the C group. Brain CAT activity was observed to be lower in the R and HS groups than the C and S groups. Our results show that stress and sulfite resulted in an increase in the lipid peroxidation process, that is accompanied by changes of antioxidant enzymes. P313.

Disease mechanisms The presence of ACD correlates roughly with the activity of the associated inflammatory disease. This observation has led to numerous studies of humoral mediators of inflammatory responses including tumor necrosis factor, interleukin-1, and the interferons. Concentrations of these inflammatory cytokines are increased in the chronic disorders associated with ACD and in animal models of ACD.9 In addition, administration of these inflammatory cytokines results in an anemia with all of the characteristics of ACD.10 These inflammatory mediators and other cytokines such as IL-6 have now all been implicated in the pathogenesis of ACD.
The pathologic hallmark of sarcoidosis is the nonnecrotizing granuloma, which consists of a compact central core of macrophage-derived epithelioid and multinucleated giant cells surrounded peripherally by lymphocytes, monocytes, and fibroblasts.16 The chemokine and cytokine pathways that regulate granuloma formation are not well understood, but TNF- is known to play a key role.11, 17 Further, TNF- production by alveolar macrophages in patients with sarcoidosis is known to be substantially elevated.18, 19 The use of the TNF- antagonists pentoxifylline and thalidomide has been reported to be effective in the treatment of refractory manifestations of sarcoidosis.1214 This article describes our experience using infliximab, a chimeric monoclonal antibody against TNF- for this purpose.
