Statistical methods The cumulative incidence of KS was estimated from all pati diagnosed with AIDS using the Kaplan Meier method. Patients had not been diagnosed with KS, either at or following an A % diagnosis, were censored at the time of their death or last follow The effect of age and time period of AIDS diagnosis on developr | of KS was assessed using a Cox model. The rates of KS diagnosi " the first, second and third years following diagnosis with AIDS ' calculated as the number of KS diagnoses divided by the numtx person-years of follow-up in that time period. Confidence intei CI ; and significance tests for these rates were calculated using rr ods based on poisson rates [7]. The effects of age, year of KS diagnosis, primary or secondary ivo, extent of KS, number of associated AIDS illnesses and CD4 count at KS on survival following diagnosis of KS were assessed using a Cox model. The effect on survival of chemotherapy treatment for KS, PCP prophylaxis and number of AIDS associated illnesses other than KS were assessed using a time dependent covariate in the Cox model.

2. Middle: restless sleep, dilated pupils, goose flesh, tremor, irritability and lack of appetite. 3. Late: an increase in early and middle signs and symptoms; an increase in heart rate and blood pressure, nausea, vomiting, diarrhea, abdominal cramps, labile mood, depression, muscle spasm, weakness and bone pain. However, even in the late phase, opiate withdrawal, which is a serious medical con dition, is rarely life- threatening. Note: These signs and symptoms are time-related, rather than related to the severity of the withdrawal as specified in the OASAS 816 regulations for Crisis Services.
Coordination between INTAL and other regional and international institutions has been increasing over time. Many activities planned for this project will be carried out in cooperation with other institutions, such as the regional integration secretariats CACM, MERCOSUR, CARICOM and Andean Community ; , WTO, UNCTAD, ITC, Asian Development Bank ADB ; , and ECLAC. Program coordination with these organizations is executed on a case by case basis, with the exception of training activities organized jointly with the WTO, which respond to a structured annual planning process!

On Friday, April 8, PanCAN collaborated with Northwestern University, University of Chicago, Evanston Northwestern Healthcare and the Michael Rolfe Pancreatic Cancer Foundation to host the first Pancreatic Cancer Symposium Chicago. Over 160 individuals attended the all-day symposium, including 34 pancreatic cancer survivors. Some of the local speakers were Dr. Mark Talamonti, MD from Northwestern University, who gave a moving talk on the doctorpatient relationship; Debbie Polidoro, RN from Evanston Northwestern Healthcare shared her knowledge on managing side effects; and Hedy Kindler, MD from the University of Chicago gave a very informative presentation on pancreatic cancer treatment combinations in clinical studies. Some of the other featured speakers were Jordan Berlin, MD from the Vanderbilt-Ingram Cancer Center who provided an overview of the current state of pancreatic cancer; Gloria Petersen, PhD from the Mayo Clinic shared what is currently known about genetics and pancreatic cancer; and Maria Petzel, RD from M.D. Anderson Cancer Center delivered information on nutritional concerns faced by pancreatic cancer patients. The day ended with a session titled, "Getting Involved in Your Community" which included Team Hope Illinois - North Coordinator, Valerie Powley and local pancreatic cancer survivors, Dr. Ed Lack and Joseph Scanlon. One caregiver commented about the Symposium, "This is a real gift to survivors and their friends and family." Another participant said, "Anyone who can attend should." And a former caregiver wrote, "I just wanted to thank you for the opportunity to attend the symposium.Although it was difficult reliving my our experiences through the words of the doctors, and the people that are still fighting the good fight, I remember how important that word HOPE was and still is." PanCAN thanks all the speakers and collaborators for making our first Pancreatic Cancer Symposium Chicago a complete success. The audio CD's and slide presentations are available through the PALS program. If you are interested in ordering copies of any of the presentations, contact medinfo pancan or call 877-272-6226 and iressa.

This paper was presented at the Policy Forum "Macroeconomic Policy Coordination and Monetary Cooperation in MERCOSUR", organized by IDB INTAL - CEBRI. Rio de Janeiro, Brazil, October 9-10, 2000 Just as there is an increase in zona reticularis mass at adrenarche, there is a decrease in zona reticularis mass and fragmentation of its cells with aging. The process closely resembles apoptosis see Figure 2 ; . This decline in zona reticularis mass is associated with the falling production and declining concentrations of DHEAS that occur with advancing age.3, 4 Testosterone during reproductive years evolves partly from peripheral intracellular conversion of DHEAS and partly from direct ovarian secretion that originates mostly from the dominant ovarian follicle of the month. Thus, a mid-cycle rise in testosterone concentrations occurs in conjunction with the luteinizing hormone LH ; surge, an event linked to increased mid-cycle sexual desire, which occurs at that time1, 2 see Figure 3 ; . After menopause, the ovary evolves from a follicleladen reproductive structure to an acyclic, androgensecreting, stroma-dominant organ that shrinks to about half its original reproductive-age size see Figure 4 ; .The post-menopausal ovary is believed to produce significant amounts of testosterone from its remnant stromal architecture. The chronically elevated levels of LH associated with menopausal years further augment this production.1, 2 Testosterone concentrations do not drop abruptly after menopause in women with intact ovaries. While the midcycle testosterone rises, basal concentrations decline only gradually. Post-menopausal oophorec-tomy, however, is associated with a sharp drop in testosterone of approximately 4050%. Not surprisingly, post-menopausal oophorectomy is associated with lost sexual desire.1, 2 Declining androgens are associated with subtle symptoms that take years to evolve and can be difficult to recognize. Depletion of androgens is not lethal, but their decline may accelerate some processes traditionally associated with aging and mortality i.e. the characteristics of reverse adrenarche that are summarized in Table 1 ; .1, 2 and isdn.

On day 12 the patient was given 10 000 IU HCG Pregnyl; Organon, Oss, Holland ; , and laparoscopy was performed 35 h later day 14 ; . Immature oocytes were aspirated using a specially designed needle Cook IVF, Queensland, Australia ; placed in the middle of the lower abdomen. Subsequently, ovarian electrocautery was carried out using a monopolar needle 40 watts ; at the puncture sites of oocyte retrieval. Sixteen oocytes were aspirated from both ovaries and collected in 10 ml culture tubes Falcon, Franklin Lakes, NJ, USA ; containing a maturation medium [diluted mixture Medi-Cult universal IVF medium Medi-Cult, Jyllinge, Denmark ; ] plus 75 IU HMG per 10 ml Menogon; Ferring, Kiel, Germany ; . Grouped oocytes were cultured as cumulus cells in Nunc 4-well dishes, 0.5 ml maturation medium per well without oil ; for 24 h. Eleven oocytes reached metaphase II, one oocyte reached metaphase I, four oocytes were degenerated, and routine ICSI was performed on each. Fertilization of seven oocytes 64% ; was assessed 16 h later by the appearance of two distinct pronuclei and two polar bodies. A total of three 2-cell embryos were transferred 48 h after ICSI, and the remaining four embryos were cryopreserved. For endometrial preparation and luteal support, the patient received one daily dose of 2 mg estradiol Progynova; Schering, Berlin, Germany ; starting at the day of oocyte retrieval and 400 mg of progesterone administered vaginally starting at the day of ICSI respectively. Two weeks after the embryo transfer the urinary pregnancy test was positive and after another 2 weeks the serum -HCG level was 3850 IU ml, and an ongoing singleton pregnancy with a fetal heartbeat was confirmed at transvaginal ultrasound examination. In October 2000 the patient had a normal vaginal delivery, the female baby weighing 3180 g. Of note, to date April 2001 ; the patient has had two regular menstrual cycles, a recent transvaginal ultrasound revealed completely normal ovaries, and the hormonal profile showed no abnormalities compared with the pre-operative findings and ivermectin.
