The prognostic value of diaphragm dysfunction, the use of diaphragm tests for follow-up and the clinical relevance of "diaphragmoriented" strategies remain to be investigated. Indeed, as vital capacity and blood gas levels are poor indicators of respiratory insufficiency [5, 19, 24, 27], such strategies could avoid leaving patients with compromised diaphragm function but otherwise subnormal respiratory test results exposed to sleep-related respiratory disorders. Meanwhile, the authors submit that dyspnoea in amyotrophic lateral sclerosis should suffice to prompt diaphragm investigations and that diaphragm dysfunction should raise the issue of ventilatory support.

Indeed, previous reports demonstrated that the mRNA levels of types I and III collagen were significantly increased in the left ventricular tissue of experimental cardiomyopathic hamsters.29, 30 Increased collagen synthesis may impair cardiac function in the development of cardiomyopathy. Therefore, a therapeutic approach to alter the fibrosis directly by means of growth factors may open a new therapeutic concept in dilated cardiomyopathy. | Levalbuterol vs racemic albuterolBefore levalbuterol was available, it does not address its use in severe, acute asthma. The Pediatric Asthma Promoting Best Practice guidelines recommend using levalbuterol 1.25 mg in patients experiencing acute broncospasm. 30 Since the dose response curve is shifted to the right in patients experiencing increased broncoconstriction, 10 it is conceivable that the patient in this case is not receiving enough 2agonists resulting in an incomplete response. After reviewing the available data, levalbuterol does not demonstrate superiority over racemic albuterol in either safety or efficacy in patients with stable asthma. In acute exacerbations of asthma, the safety and efficacy of levalbuterol over racemic albuterol has not been well established; however, there is no evidence to suggest that levalbuterol is not at least as safe and efficacious as racemic albuterol. Indeed, some preliminary studies suggest it may be superior to racemic albuterol in this situation; however, this needs to be more thoroughly studied before a conclusion can be made. Additionally, the cost of Xopenex has been reported to be 3531, 32 times higher than racemic albuterol; however, this difference will depend on the institution and contract agreements. Considering the lack of evidence in using levalbuterol in acute asthma exacerbations and that levalbuterol is on a restricted formulary due to cost considerations, it is reasonable that the clinical pharmacist recommends that the patient be switched to racemic albuterol and levonorgestrel.
