In vitro experiments with isolated ruminal epithelia have confirmed these results and demonstrate that the so-called K + - or PD-insensitive component of Mg2 + uptake is stimulated specifically 23 ; . After excluding a Mg2 + 2H + antiporter as the underlying mechanism, we have hypothesized the existence of a Mg2 + -anion cotransport 36 ; . The results obtained in the high-NaCl medium, viz., 1 ; that butyrate alone is not able to stimulate Mg2 + uptake 2 ; that the rate of Mg2 + uptake is the same in HCO3--buffered media and HCO3--buffered media with additional butyrate, have led us to presume that the Mg2 + influx is coupled to HCO3- uptake via a Mg2 + -2HCO3cotransport. Such a transport system has been shown by Gnther et al. 14 ; in Yoshida ascites tumor cells. However, the existence of an Mg2 + -2HCO3- cotransport is strongly opposed by the finding that the stimulating effect of HCO3- is completely abolished after reduction of the extracellular [Cl-]. In the experiments performed by Gnther et al. 14 ; , even the total removal of Cl- from the extracellular solution had no negative influence on the Mg2 + uptake as long as HCO3- was present. Another result mitigating against a Mg2 + -2HCO3- cotransport is the significant reduction of Mg2 + influx in the HCO3--buffered high-KCl medium.

Crohn's disease The safety and efficacy of infliximab were assessed in 108 patients with moderate to severe, active Crohn's disease Crohn's Disease Activity Index CDAI ; 220 400 ; in a randomised, doubleblinded, placebo-controlled, dose-response study. Of these 108 patients, 27 were treated with the recommended dosage of infliximab 5 mg kg. All patients had experienced an inadequate response to prior conventional therapies. Concurrent use of stable doses of conventional therapies was permitted, and 92% of patients continued to receive these medications. The primary endpoint was the proportion of patients who experienced a clinical response, defined as a decrease in CDAI by 70 points from baseline at the 4-week evaluation and without an increase in Crohn's disease medications or surgery for Crohn's disease. Patients who responded at week 4 were followed to week 12. Secondary endpoints included the proportion of patients in clinical remission at week 4 CDAI 150 ; and clinical response over time. At week 4, following a single dose of study medication, 22 27 81% ; of infliximab-treated patients receiving a 5 mg kg dose achieved a clinical response vs. 4 25 17% ; of the placebo-treated patients p 0.001 ; . Study objective: To determine the efficacy of methotrexate, vinblastine, and platinum chemother apy in patients with diffuse unresectable malignant mesothelioma. Design: Patients with histologically confirmed malignant mesothelioma were evaluated for treatment with methotrexate, vinblastine, and cisplatin chemotherapy. If the patient had preexisting hearing loss or neuropathy, or was significantly disabled eg, spending greater than half of the day in bed or a chair ; , cisplatin therapy was withheld. Setting: All patients were initially evaluated at the University of Washington Medical Center and received chemotherapy at the University of Washington or in the community. Interventions: Between 1990 and 1994, 17 patients received this chemotherapy. Ten patients received cisplatin, 100 mg m2 IV on day 1, methotrexate, 30 mg m2 IV on days 8, 15, and 22, and vinblastine, 3 mg m2 IV on days 8, 15, and 22, in 28-day cycles. One patient had carboplatin substi tuted for cisplatin due to preexisting hearing loss. Six patients received weekly methotrexate and vinblastine at the same doses without platinum. Measurements and results: Nine of the 17 53%; 95% confidence interval [CI], 28 to 77% ; patients responded, including two complete remissions, two partial remissions, and five regressions. Median time to progression is 8 months. The median survival time for all patients is 14 months. Projected 2-year survival is 35% 95% CI, 12 to 60% ; . Conclusions: Although the number of the patients in this study is small, the response rate and pro jected 2-year survival of 35% are better than thoseintypically reported for unresectable malignant mesothelioma. Further investigation is warranted confirmatory trials.

Enable a truly strategic discussion of different budget options against scientific opportunities; progress and impact of activities; and the evolving research environment and public health needs. Previous JCB working groups on budget and governance have also suggested that, in between sessions, the Secretariat should provide information on budget status and any changes in stakeholders' commitments so as to facilitate JCB's response and support to TDR. This is important and the ERC supports the recommendation, with the proviso that these updates are brief and informative, and do not consume even more of the time of TDR staff than at present. The proposal to hold JCB sessions outside Geneva every other year was also made to redirect TDR's focus more effectively to the needs of DECs. The Secretariat has recently organized one-day briefing sessions, immediately before the JCB, for newly designated regional members of JCB. The ERC recommends institutionalizing these improvements, and further recommends bringing JCB even closer to countries' needs by fostering mechanisms that would give DECs a higher profile and more active role in JCB, e.g. in agenda setting and making presentations regarding regional needs.

Within the main river valley, the FPA boundary is intended to coincide with the bluff edge or functional ridgeline behind which development would be relatively unobtrusive on the open space character of the valley floor. This FPA is identified by the JPA as the area where planning and acquisition efforts for the Park are to be directed. The boundaries of the FPA were first established in 1981 by the Pekarek Study, and at that time the planning area extended only to Lake Sutherland. The FPA was extended eastward in 1988 by the SANDAG San Dieguito River Valley Regional Open Space Park Plan Executive Committee the predecessor to the JPA ; to include the entire San Dieguito River drainage, as well as several major tributary canyons. Boden Canyon, a 1, 500-acre area located immediately to the west of Pamo Valley, was added to the FPA by the JPA in 1990. In 1993, the JPA once again expanded the FPA boundary, this time eastward to incorporate the entire 11, 000-acre Rutherford Ranch, which includes the eastern and western slopes of Volcan Mountain as well as a portion of the San Felipe Valley. The inclusion of the Rutherford Ranch into the FPA establishes a link from the ocean to the desert and the Anza Borrego State Park. The FPA boundary is not intended to establish a static demarcation between potentially developable and undevelopable land. It serves instead to identify those areas where improper development could significantly impact the existing character of the river valley. In addition, it is within the FPA that acquisition funds should be directed, and where park amenities such as the "Coast to Crest Trail" will be established. As more specific planning efforts are undertaken, in the form of master plans for the individual landscape units within the Park planning area, it may become apparent that the FPA boundary should be expanded or constricted to more accurately reflect the viewshed of the river valley. Ownerships Within The Focused Planning Area The properties included within the FPA represent both publicly and privately owned land. Approximately 60 percent of the area is publicly owned, with the largest percentage owned by the City of San Diego Water Utilities Department. The remaining areas of publicly owned land include lands owned by the U.S. Forest Service Cleveland National Forest ; , the Bureau of Land Management, the State of California, the County of San Diego, the cities of Del Mar and Escondido, and the Olivenhain Municipal Water District. Approximately 800 acres of land located east of Lake Sutherland are owned by the Mesa Grande Band of Mission Indians. Privately owned lands, which make up the remainder of the planning area, occur throughout the FPA and range from one-acre ownerships to ownerships in excess of 1, 000 acres and midodrine.
