The endocarditis lesion was composed of fibrinous layers with necrotic cell debris in between the concentric layers and an accumulation of mononuclear inflammatory cells in the tissue of the atrial wall where the lesion attached. The presence of an endocarditis lesion together with multifocal to coalescing necrotising encephalitis lesions was suggestive of an infectious etiological agent, that did not primarily generate a purulent inflammation. But blood agar plates inoculated with aseptically collected samples from the lesions remained apparently sterile after 48 hours aerobic and microaerophilic incubation at 37C. ZN stained, Gram stained and PAS stained fixed tissue sections revealed no acid-fast, gram-positive bacteria or fungi.
