A key point to remember is that acute pulmonary radiation damage, radiation pneumonitis, can present anywhere in a spectrum of pathologic conditions. RECENT EVIDENCE SUGGESTS that the over-thecounter medication, phenylpropanolamine PPA ; , can lead to stroke in certain individuals.1-2 Less severe complications include headaches, seizures, psychiatric disturbances and cardiac arrhythmias.3"10 Phenylpropanolamine is found in over-the-counter diet preparations" and street "look-alike" pills, ' 8 - 9 most of which contain PPA in combination with caffeine. "Look-alike" pills are made to look like prescription drugs, but do not contain the same ingredients. The purpose of this experiment was to determine if the PPA caffeine combination could lead to stroke and or an elevation in blood pressure in normotensive and hypertensive rats. The PPA caffeine dose was calculated from the allowed dose for humans, taken x 6, and calculated to an equivalent dose weight for the rats. There were two major experimental groups: 1. those repetitively injected with PPA caffeine that were later examined histologically for brain abnormalities; 2. those in which blood pressure was recorded in awake animal before and after a single PPA caffeine injection. Within each experimental group, half of the animals were drug treated and the other half were saline treated. Methods Seventy-one rats were used for these experiments. Three different animal strains were studied: SpragueDawley normotensive rats Nt ; , Harlan Laboratories, Indianapolis, Indiana ; , spontaneously hypertensive rats SHR ; Taconic Farms, Inc., Germantown, N.Y. ; and stroke prone SHR SP-SHR ; local colony obtained originally from Carl Hansen at NIH ; . Educational Objective: Differentiate between rupture of a popliteal cyst and thrombophlebitis. The probable diagnosis in this man is a rupture of a popliteal cyst Baker's cyst ; into the calf, an event that can mimic thrombophlebitis. It is important that the diagnosis be made because anticoagulation in such a patient can lead to excessive bleeding in the calf muscles and adjacent tissues. Ultrasonography is the best and least expensive technique for displaying a popliteal cyst and its rupture or extension into the lower leg; it will also identify deep venous thrombosis. A helpful sign for diagnosis, when it is found, is the appearance of a crescentic ecchymosis beneath one of the malleoli of the ankle. Before recognizing the usefulness of ultrasonography in making the diagnosis, arthrography radiographic injection into the knee ; was used. If ultrasonography results are equivocal, MRI and CT would be additional techniques for imaging a popliteal cyst. Venography is not indicated. Although joint aspiration and injection of glucocorticoids into the joint would not harm the patient, it would not help in discerning between phlebitis and rupture of a popliteal cyst. Rarely, both thrombophlebitis and a rupture of a popliteal cyst can occur concurrently. When the diagnosis of a ruptured popliteal cyst is made, intra-articular glucocorticoids can be used to suppress the inflammation and decrease the volume of fluid in the joint. Popliteal cysts develop when excessive intra-articular pressure causes a posterior herniation, often with formation of a one-way valve effect, whereby fluid forced into the cyst cannot return to the joint space. Rarely, synovectomy of the knee is necessary for recurrent cysts. Surgical excision of popliteal cysts is rarely done because the rate of recurrence is high.

Isobolographic analysis is a technique that utilizes multiple doses in combination that aims to produce the same effect and help in determining whether combinations are additive, subadditive or supra additive. It is very common and typical to use drug combinations in chemotherapy or in combination with radiation and surgery. Drugs with unique mechanisms of action are used to attack aberrant cells via different pathways. The sympathomimetics amphetamine, ephedrine and phenylpropanolamine each interact with presynaptic terminals and cause a release of norepinephrine to produce a response. Compared to a saline control, each of these compounds decreases food intake. Tyrosine supports catecholamine synthesis. When tyrosine is combined with such sympathomimetics, it enhances the effectiveness of these compounds in decreasing food intake. Tyrosine also potentiates the analgesic activity of opioids such as morphine. Tyrosine, however, does not potentiate the respiratory depressant effects of opioids. In testing a number of different dietary supplement products, a wide variety of content is found. With ephedrine, different isomers have different pharmacological effects. It is difficult to design experiments examining interactions of compounds when the contents of the test products are not known definitively. Future research should include rigorous quantitative studies to confirm suspected synergistic or potentiating activity. Safety and efficacy must be monitored e.g., blood pressure, gastric effects, respiratory depression ; , and uniformity of test articles must be insured. Finally, compounds must be tested in real-life use patterns, such as with the recent report involving aspirin and ibuprofen. Discussion.

In this study, we have used time-lapse video cinematography to study fertilization in 50 human oocytes that had undergone intracytoplasmic sperm injection ICSI ; . Time-lapse recording commenced shortly after ICSI and proceeded for 1720 h. Oocytes were cultured in an environmental chamber which was maintained under standard culture conditions. Overall, 38 oocytes 76% ; were fertilized normally, and the fertilization rate and embryo quality were not significantly different from 487 sibling oocytes cultured in a conventional incubator. Normal fertilization followed a defined course of events, although the timing of these events varied markedly between oocytes. In 35 of the 38 fertilized oocytes 92% ; , there were circular waves of granulation within the ooplasm which had a periodicity of 2053 min. The sperm head decondensed during this granulation phase. The second polar body was then extruded, and this was followed by the central formation of the male pronucleus. The female pronucleus formed in the cytoplasm adjacent to the second polar body at the same time as, or slightly after, the male pronucleus, and was subsequently drawn towards the male pronucleus until the two abutted. Both pronuclei then increased in size, the nucleoli moved around within the pronuclei and some nucleoli coalesced. During pronuclear growth, the organelles contracted from the cortex towards the centre of the oocyte, leaving a clear cortical zone. The oocyte decreased in diameter from 112 to 106 m P 0.0001 ; during the course of the observation period. The female pronucleus was significantly smaller in diameter than the male pronucleus 24.1 and 22.4 m respectively, P 0.008 ; and contained fewer nucleoli 4.2 and 7.0 respectively, P 0.0001 ; . After time-lapse recording, oocytes were cultured for 48 h prior to embryo transfer or cryopreservation. Embryo quality was related to fertilization events and periodicity of the cytoplasmic wave, and it was found that good quality embryos arose from oocytes that had more uniform timing from injection to pronuclear abuttal and tended to have a longer cytoplasmic wave. In conclusion, we have shown that time-lapse video cinematography is an excellent tool for studying fertilization and early embryo development, and have demonstrated that human fertilization comprises numerous complex dynamic events. 532.
