Table 1. Thyroid Function Tests in a Kindred with RTH. 2-Chlorodeoxyadenosine 2-CdA ; yields high complete remission CR ; rates in patients with hairy cell leukemia HCL ; . In an effort to detect minimal residual disease, w e studied two B-lineage antibodies, L26 and MB2, and a Tlineage antibody, UCHL-1, in fixed marrow core biopsies from 3 4 patients with HCL before and after 2-CdA. Before therapy, hairy cells exhibited intense cytoplasmic membrane reactivity with L26 and strong intracytoplasmic reactivity with MB2. UCHL-1 did not react with hairy cells. Thirty-one patients were assessable 3 months after therapy. Five of 24 21% ; patients in CR by routine evaluation had residual HCL detected by immunostaining. Four of these 5 patients have been reevaluated at 1 year. One patient relapsed by routine evaluation, 2 remained positive by immunostaining alone, and 1 patient became negative by immunostaining. A total of 19 patients have been evaluated at 1 year. Only 1 additional patient has become positive by immunostaining alone. lmmunostaining using the B-lineageantibodies highlighted the presence of hairy cells with preservation of morphology. This assisted in quantifying the extent of disease, particularly when hairy cells were interstitial and blended with surrounding hematopoietic tissue, when hairy cells were present in hypocellular marrows, when hairy cells were spindle-shaped, and when marrows were markedly fibrotic. Because immunostaining can be easily performed on routinely processed marrows, it is an attractive method to detect minimal residual disease. Our data suggest that some patients in apparent CR after 2-CdA may have minimal residual disease. Patients will need to be observed prospectively to determine if residual disease will be predictive of relapse. 0 1993 by The American Society of Hematology.

Rhinocort is an anti-inflammatory steroid medication. OBSERVATIONS OF THE SCIENTIFIC SUB-COMMITTEE O. Eng. ; 1. 2. The Sub-Committee examined three questions posed by the Harmonized System Committee concerning "Rozmrazovac 80 C" paragraph 3 of Doc. 41.671 ; . Concerning the first question, i.e., whether the substances, other than water and ethanol, in the preparation were simply denaturants or whether they rendered the preparation something other than denatured alcohol of heading 22.07, there was consensus that they were not simply denaturants. Opinions were divided, however, as to the role of these substances in the preparation. In this connection, the Delegate of Finland took the view that the presence of a surfactant was not typical of de-icing preparations and that, instead of being a de-icer, "Rozmrazovac" was suitable for use as a windscreen washing preparation. As the product contained a surface-active agent, it was, in his view, a product of heading 34.02. The Delegate of Canada supported this view, noting that the high ethanol concentration allowed the washing preparation to be used at low temperatures. He further noted that deicing preparations usually contained little or no water, but instead consisted almost entirely of alcohols or mixtures of alcohols with glycols; such preparations were sprayed directly on the windscreen to facilitate the removal of ice. Windscreen washing preparations, on the other hand, generally contained about 50 percent alcohols, the remainder being water, surfactants and a blue dye; these preparations were kept in the windscreen washer reservoir year round and were used for cleaning the windscreen in hot or cold weather. He added that the ethylene glycol served to prevent the solution from freezing during cold weather. The Delegate of Norway supported the Finnish and Canadian views and noted that, in the end, de-icing fluids served to facilitate the physical scraping of ice from the windscreen. The Delegate of the United States expressed his opinion that, though the products were probably not deliberately denatured, they were nevertheless not potable. He agreed that the added substances were present for purposes other than as denaturants. He felt that, as presented i.e., without further dilution with water ; , the products could be used as de-icing fluid. The Delegate of Switzerland indicated that surface-active agents were typically found in de-icing fluids, though such preparations generally contained two or more alcohols, not just ethanol. The Delegate of Germany noted that the purpose of the surfactant was to improve the wetting of the surface to give better contact between the de-icer and the ice. He also felt that ethylene glycol served to reduce the volatility of the ethanol in the preparation and rifabutin. Diagram Showing Scope of Gearing Up mapped on to the Conceptual Framework1 The programme was completed in March 2006 and was successful in achieving the majority of i ccess m easu res and in doing so received very positive feedback from external inspectors. ts su "The Gearing Up programme is well organised. The programme has been driving the internal change agenda, the programme is clearly defined, well managed and monitored through the programme board and shows good practice in project management of the work streams. It is successfully bringing about attitudinal an d org an i on e."2 sati However at its close the programme was perceived by some senior managers and elected members not to have delivered the desired results: "A t a ressure on th e stan d i an oth er assessm en ts, the m g l programme was perceived by some to have not delivered the required depth and pace of cu l ral ch an g erform an ce i rovem en t." 3 tu m This paper seeks to reflect the learning from the Gearing Up programme and to consider both the factors that contributed to the successful delivery of the programme and those which meant that the programme was perceived by some not to have delivered the organisational change that was hoped for by some key stakeholders. The nature of this paper is experiential; however the analysis is presented within the context of the conceptual framework of public management developed by Paul Joyce and within a wider consideration of organisational development theory. The structure of the paper reflects John Kotter 4 model of reasons why change efforts fail as this provides a useful structure for s analysing the strengths and weaknesses of the Programme.

The primers for the Cterminal fragment 875 bp ; were: forward primer, 5 -ACT GTC TTT GCA AGC GAG TT-3 ; and reverse primer, 5 -GTG TCA TGG GAT TGG AAT GC-3 16 ; . PCR was performed in a 50- L reaction mixture containing 100 ng genomic DNA, 10 mmol L Tris-HCl pH 8.3 ; , 1.5 mmol L MgCl2, 50 mmol L KCl, 0.01% gelatin, 200 mol L deoxy-NTP, 1 U PrimeZyme polymerase Biometra, Gottingen, Germany ; , and 10 pmol of each primer. After an initial denaturation of 3 min at 95 C, samples were subjected to 30 cycles of 30 s and 1 min at 72 C, followed by a final extension step of 6 min at 72 C. PCR products were purified by polyethylene glycol precipitation 17 ; , and double stranded sequencing of PCR products was performed with universal dye primers Applied Biosystems, Weiterstadt, Germany ; and Thermosequenase.

Maintenance of Anesthesia Although recent studies propose the use of specific anesthetic maintenance agents, there is insufficient evidence at this time to conclude improved safety and outcomes by this approach 42 46 ; . Limited information is available regarding intraoperative fluid replacement and maintenance of normothermia in severely obese patients 47, 48 ; . However, intraoperative mild hypothermia, common during open and laparoscopic bariatric procedures, improved with active warming in at least one study 48 ; . Intraoperative euvolemia and normothermia may aid in physiological homeostasis and adequate organ perfusion. The effects of the pneumoperitoneum during laparoscopic WLS surgery on pulmonary physiology and hemodynamic parameters have been investigated 49 51 ; . Perioperatively, blood pressure BP ; assessment may be challenging because of the patient's body habitus and the inability to correctly position a noninvasive BP cuff on a conically shaped arm. However, there is no evidence to suggest reduced patient safety and outcome due to the common practice of using standard noninvasive BP equipment at alternate sites such as the patient's forearm. There are no data showing that routine invasive hemodynamic monitoring improves safety and outcome in WLS patients. Recommendations Categories B and D.
