Smoking is the main external preventable cause for the development of chronic obstructive pulmonary disease COPD ; . Smoking cessation and long-term home oxygen treatment for end stage disease ; are the only treatments that affect the prognosis of COPD [1]. Nicotine replacement therapy NRT ; approximately doubles the 1-yr success rate in smoking cessation compared with a placebo [3]. As smoking involves both pharmacological and psychological factors receiving some degree of behavioural support is important, and as suspected the success rate increases with increasing intensity of behavioural therapy [4]. The highest success rates with NRT have been achieved in special smoking cessation clinics with 1-yr success rates of 2535% as compared to success rates of 10% achieved in general practice with nicotine patches, however, NRT also doubled the success rate when compared to a placebo [58]. Nicotine patches, a fixed dose delivery system, which "infuses" approximately 1 mg of nicotine per h through the skin, are easy to use, and have shown an effect over a placebo in more than 20 studies even with minimal instruction in use [9]. Nicotine inhalers which deliver most of the nicotine into the mouth and throat, can be used ad libitum but this selftitration has both pros and cons. Our observation of changes in CD38-expression levels in T cells over time in 10% of patients cells might represent a caveat for the use of these parameters for risk assessment in B-CLL. However, the same frequency of changes was observed for CD38-expression levels in B-CLL cells in our study and by others, 8 suggesting similar values as prognostic factors. From our study, it can be concluded that combined analysis of CD38 in B-CLL cells and T cells for identification of risk groups in male B-CLL might be superior to the use of either parameter alone. For clinical risk evaluation in B-CLL, it is unlikely that one single parameter being CD38-expression levels of T cells or others ; will be enough for accurate prediction of clinical outcome, considering the high complexity of this disease. However, by adding novel independent risk parameters such as CD38 expression in T cells which indeed can be assessed easily in clinical routine ; to established ones, as we propose from the results of our study, the predictive power of molecular profiling should significantly increase. The clinical value of CD38 expression in T cells is also underlined by the fact that multivariate analysis revealed CD38 expression of B-CLL cells and T cells and Ig VH mutational status as independent parameters, whereas important risk factors such as Zap-70, genomic aberrations, and Rai stage became insignificant in this model Table 3 ; . The prognostic power of CD38 expression in T cells certainly has to be confirmed in prospective studies. The negative impact of CD38 T cells on the course of disease might also become an important issue in situations when autologous T cells are expanded and or stimulated for immune therapies and underlines the need for further work on the biologic background of these sex-specific interactions.
