Epirubicin vinorelbine combination therapy

Background Foot ulceration is thought to affect 15% of people with diabetes at some time in their lives. Objectives To assess the evidence for debridement diabetic foot ulcers. Search strategy Searches of electronic databases, hand searching journals, bibliographies and identification of unpublished work by contacting experts in the diabetic foot. Selection criteria Randomised controlled trials RTCs ; evaluating of debridement in the treatment of diabetic foot ulcers. The outcome had to include either complete healing or rate of healing. There was no restriction on articles, on language or publication status. Data collection & analysis Data extraction and assessment of study quality were undertaken by one reviewer and checked by an Editor of the Wounds Group. Main results 5 RCTs of debridement were identified; 3 RCTs assessed the effectiveness of a hydrogel as a debridement method, 1 RCT evaluated surgical debridement and 1 RCT evaluated larval therapy. Pooling the 3 hydrogel RCTs suggested that hydrogels are significantly more effective than gauze or standard care in healing diabetic foot ulcers absolute risk difference 0.23; 95% CI: 0.10, 0.36 ; . Surgical debridement and larval therapy showed no significant benefit in these small trials. Other debridement methods such as enzyme preparations or polysaccharide beads have not been evaluated in RCTs of people with diabetes. Reviewers' conclusions There is evidence to suggest that hydrogel increases the healing rate of diabetic foot ulcers. More research is needed to evaluate the effects of a range of widely used debridement methods and of debridement per se. This review should be cited as: Smith J. It is recommended that vinorelbine be discontinued if moderate or severe neurotoxicity occurs during treatment. Patients. The impact that carboplatin had in these results is therefore difficult to evaluate. The cisplatin versus carboplatin CISCA ; meta-analysis presented at ASCO 2006, based on individual patient data from 2, 968 patients in nine trials that compared a cisplatinwith a carboplatin-based regimen in advanced stage disease, is of value in making this assessment [11]. This study differed from the earlier Hotta et al. [12] and Zojwalla et al. [13] meta-analyses comparing cisplatin with carboplatin, which were based on published information and showed a slight superiority of cisplatin. In the meta-analysis by Hotta et al. [12], cisplatin effected a statistically superior overall survival benefit in comparison with carboplatin, albeit a small difference, only when combined with newer-generation chemotherapy agents HR, 1.106; 95% CI, 1.005 1.218; p .039 ; . In the CISCA analysis, response rates were 33% with cisplatin and 26% with carboplatin, which correlated with an HR for response of 1.37 95% CI, 1.16 1.62; p .001 ; . The HR for survival favored cisplatin, at 1.07, but this was not statistically significant 95% CI, 0.99 1.15; p .10 ; . The median survival duration for the cisplatin-treated patients was 9.1 months, versus 8.4 months for the carboplatin-treated patients p .101 ; . Subset analyses, however, did show a statistically significant.

There is evidence suggesting that Akt promotes breast cancer cell survival and resistance to chemotherapy, trastuzumab, and tamoxifen. Rapamycin is a specific mTOR antagonist that targets the Akt PI3K pathway and blocks the downstream signaling elements, resulting in cell cycle arrest in the G1 phase Carraway, 2004 ; . Rapamycin and rapamycin analogs have emerged as promising antitumor drugs for many cancer types, including breast cancer. When combined with other chemotherapeutic agents, rapamycin and rapamycin analogs have been reported to increase the efficacy of a variety of cytotoxic agents, including cisplatin, doxorubicin, 5-FU, and cyclophosphamide Geoerger, 2001 ; . Mondesire et al reported that in vitro rapamycin has a synergistic effect with paclitaxel, carboplatin, and vinorelbine and an additive effect with doxorubicin and gemcitabine. Furthermore, they showed that rapamycin in combination with paclitaxel leads to a significant reduction in tumor growth in vivo in a rapamycin-sensitive xenograft model. Their results demonstrated that rapamycin might be able to at least partially overcome resistance to paclitaxel and carboplatin in HER2 neu-overexpressing cells, suggesting a potential approach to these poorly behaving tumors. On the other hand, cell lines that were resistant to the growthinhibitory effect of rapamycin were also resistant to rapamycin-mediated chemosensitization. This suggests that combination therapy of cytotoxic agents with rapamycin may be effective in patients selected for aberrations in the PI3K Akt pathway Mondesire, 2004 ; . In the next few years, and as trials with targeted agents mature, their role will be further defined and their mechanism of action will be more elaborated. This may help identify patients who will benefit the most from these novel therapeutic approaches.
